What you see on the PDF copy is what I was given from Olive Garden. I had to white-out the header information because it had my Government email address and we aren't allowed to have that kind of information included on public forums. Sorry I couldn't be of more help to you...

I wrote to you some time back about getting a full copy of your email so that we may be able to use it here in Washington State. I will block out any personal info. The info you posted on the board is pretty vauge as far as contact info. I prefer to post a PDF of the email with their return contact and email ID info.
